SummaryJOB SUMMARY: Under the clinical direction of the physicians and mid-level providers coordinates pre and post operative care for patients to achieve desired patient and program outcomes for identified patients and patient populations. Functions as a patient navigator to facilitate appropriate access to care and assesses patient care delivery process, systems, clinical pathways and care management data to recommend improvements in quality, service, cost, safety, etc. Consults with other key staff on specialized care plans and multi-disciplinary assessments and services as a liaison in the discharge planning process. Provides patient education and assists patients with questions regarding their care in collaboration with the physicians and mid-level providers. At all times operates within the strict scope of practice as defined for an LPN.REQUIREMENTS: Experience: Less than one year. Education Required: High School / GED Preferred. Associate's Degree in Nursing Licenses And Certification: Required: Basic Life Support.
