Job Description Job Description Clinical Pharmacist Schedule: Shift: 4 Days/Week, 10-Hour Shifts Hours: Rotating – 6:00AM–4:30PM, 7:00AM–5:30PM, or 12:30PM–11:00PM Weekend Requirement: Every Other Weekend (2 x 10-hour shifts) Urgency: Quick Start Job Description & Responsibilities: General Summary: Pharmacists are responsible for filling drug orders, monitoring patient drug therapies, providing drug information, and delivering pharmaceutical care with compassion, efficiency, and excellence. Core Responsibilities: Operational Duties: Prepare and dispense drug orders per physician request Interpret and transcribe drug orders into the EMR accurately Compound non-sterile/sterile, chemo, and TPN preparations Issue controlled substances per legal/institutional policies Supervise and verify work of pharmacy technicians Maintain pharmacy licensure and CE records Support ambulatory care programs (e.g., med reconciliation, anticoagulation clinics) Clinical Duties: Monitor for interactions, contraindications, and dosing appropriateness Provide pharmacokinetics, renal dose adjustments, and drug education Sustain formulary compliance and manage substitutions Participate in interdisciplinary rounds, stewardship, and QI initiatives Report adverse drug reactions and maintain documentation Conduct clinical interventions and report outcomes Qualifications: Education: Doctor of Pharmacy or BS in Pharmacy (Required) Experience: Hospital experience preferred Licenses/Certifications: Active Pharmacy Registration in State of Practice State Pharmacist Licensure Required BLS & ACLS (American Heart Association) Company Description LanceSoft is rated as one of the largest staffing firms in the US by SIA.
