Job Description:
MedStar Health, a leading not-for-profit healthcare organization
in the D.C.-Maryland-Virginia region, is seeking a dedicated CTO
Transition of Care LPN Care Manager to join our vibrant team. This
role is pivotal in ensuring seamless transitions for our patients
between care settings, aiding in recovery and maintaining the
continuum of care. The ideal candidate will work closely with
multidisciplinary teams to manage and optimize patient care plans,
ensuring the highest standards of healthcare are maintained during
transitions. Coordinate effectively with healthcare teams across
different service areas to ensure comprehensive transition plans.
Evaluate patient needs and health status to develop detailed care
management plans. Monitor patient progression and adjust care plans
as needed. Educate patients and families about treatment plans and
healthcare resources. Maintain detailed records of patient
interactions, treatment plans, and outcomes. Ensure compliance with
healthcare regulations and standards during transitions. Strong
clinical judgment Excellent coordination and communication skills
In-depth knowledge of care management and patient care Ability to
work effectively in a multidisciplinary team Proficiency in
electronic medical record systems Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Current LPN
licensure in the state of employment and CPR certification.
